View Point Health is a community behavioral health center utilizing a team of psychiatrists, nurses, licensed counselors and social workers, as well as other professionals to provide services to individuals who need treatment and support to cope with mental illness, substance abuse, and intellectual and developmental disabilities. View Point Health serves uninsured, underinsured, low-income Medicaid, Medicare, war veterans and some private insurance across multiple locations with a full continuum of behavioral health services and support.

View Point Health is partnering with community social service providers in Gwinnett, Rockdale and Newtons counties to assist with mental health and case management needs. The partnership aims to embed a View Point Health staff with a community partner to help engage individuals with behavioral health challenges and help them develop an action plan to be successful in their recovery.

The primary purpose of the program is to screen participants in community partner programs for behavioral health challenges. Those individuals with behavioral health needs will be paired with the VPH team to create and implement a strengths-based plan of care. The primary goal of the program is to assist the individual by identifying specific behavioral health needs, promote recovery, and assist the individual achieve their goals in the community.

The Case Manager plays a critical role in coordinating care and services for individuals with mental health challenges. Working as part of a multidisciplinary team, the Case Manager assesses client needs, develops individualized service plans, and connects clients to appropriate community resources. The goal is to promote stability, recovery, and independence in clients lives through consistent support, advocacy, and service coordination.

Under broad supervision, provides a broad range of social services to patients/clients/consumers and their families. Conducts patient/client/consumer intakes, participates in treatment team planning, and performs crisis intervention. Assists in dealing with personal and social problems. May provide supportive counseling to consumers and families and/or serve as a liaison for social services. May perform case management duties. Performs lead worker functions.

Duties & Responsibilities:
Conduct comprehensive intake assessments and psychosocial evaluations. Develop, implement, and regularly update individualized treatment/service plans. Coordinate and monitor services including mental health treatment, substance use services, housing, medical care, employment, and other supports. Advocate for client access to resources, services, and benefits. Maintain regular contact with clients via home visits, office appointments, and phone check-ins. Facilitate access to emergency interventions when needed. Participate in case conferences, team meetings, and supervision sessions. Maintain accurate and timely documentation in compliance with agency, state, and funding requirements. Collaborate with internal clinical staff, external providers, families, and legal representatives to ensure integrated care. Educate clients and families about mental health conditions, treatment options, and available support. Crisis intervention and de-escalation as needed. Perform other related duties.

Minimum Qualifications:
High school diploma/GED and three (3) years of job-related experience in a social services-related position; or one (1) year of experience at the lower level Social Svcs Tech 2 (SST011) or equivalent position.

Preferred Qualifications:
Bachelors degree in social services field. Experience working with the Severe and Persistently Mentally Ill population, ACT teams, or individual identified as being frequent users of ERs, Psychiatric Hospitals, Crisis Stabilization Units, and the criminal justice system Associate licensed as a mental health professional (LAPC or LMSW) Case management experience Experience working in a highly stressed, fast-paced environment with a very emotionally and physically changing population Experience working in multi-disciplinary teams Ability to multi-task and manage time Bilingual skills are a plus (Spanish)

Requirements/Competencies:
