Provide Target Case Management and Wraparound Facilitation to persons assigned to caseload.
Facilitate meetings with treatment/wraparound teams.
Develop comprehensive treatment plans for persons in service.
Coordinate, monitor and/or complete all waiver paperwork, releases, and assessments.
Work hours are flexible to meet the needs of the person and families served.
Monitoring of progress of treatment plans and interventions and making changes as necessary.
Coordination of care with community partners.
MINIMUM QUALIFICATIONS:
Age 21+ years old and have a BA/BS degree with an emphasis on Mental Health/human services or equivalent experience (ie, 4 years work experience, or 2 years schooling + 2 years work experience in related field).
Excellent interpersonal and teamwork skills
Excellent organizational and time management skills
Interest and investment in working with children who experience Severe and Emotional Disturbance (SED) and their families.
Have completed or will complete any required training within six months of employment.
Must have a current and valid Kansas driver's license.
