Community Health Worker Supervisor (Finance)
The person that is hired for this position, will be able to assist with implementing new policies and procedures as well as expanding the knowledge of community health workers' skill set. Other attractions for this position is being able to participate in mentoring, professional development and recruitment selection of identifying other qualified applicants.
Preferred skills are community health worker instructor certification, excellent written and verbal communication skills, competent presentation and training skills. Candidate should be proficient with understanding data, community health worker workflow trends, Epic workbench reports, and being able to educate care team members.
Schedule: Monday - Friday, 8am - 5pm

Provides professional and supervisory work involving the coordination between medical providers, primary care teams and surrounding communities.
Position Key Accountabilities:
1. Provides overall coordination and management of Community Health Worker (CHW) programs to assure the achievement of its goals and objectives. Develops and modifies CHW programs and activities to assure quality patient services.
2. Participates in regular, systematic review of cases and their coordination for environmental, social, community and economic resources. Includes directly supervising staff in clinics, and participating in quality improvement meetings and medical record reviews.
3. Directs community resources to support services available in the community for various clinics: geriatric, pediatric, chronic diseases, adult primary and specialty.
4. Supervises and reviews activities, service plans, and implementation of population health programs to ensure adequate patient care coverage in an effective manner while strictly adhering to policies and procedures.
5. Continuously expands Community Health Worker knowledge and understanding of community needs, resources, services and programs available by providing continuing education opportunities.
6. Responsible for multiple locations and travels to service locations as assigned.
7. Plans for CHW coverage for special projects or where there are long term vacancies.
8. Maintain confidentiality per HIPAA guidelines in regards to patient information.
9. Assists with the recommendation and implementation of new policies and procedures to more effectively utilize available staff and capabilities.
10. Interacts with peer positions in other public service agencies and acts as a representative on interdepartmental committees and acts as a community liaison with referral agencies. Interfaces with appropriate public service and clinic staff to assure a cohesive working relationship, and to provide comprehensive social and community referrals. Assists administration in coordinating community actions, special administrative and clinical projects, and in implementing policies for community health work activities.
11. Ensures compliance with departmental policies and procedures and all licensing bodies. May monitor the medical records audits and implements procedural changes to correct deficiencies.
12. Manages Human Resources activities for direct reports in regards to: recruiting and selection, hiring and termination, training, professional development, mentoring, counseling, performance evaluations, and salary planning.
13. Performs other duties as assigned.
Certification/Skills:
Texas Department of State Health Services (DSHS) - Certified CHW
Texas Department of State Health Services (DSHS)- Certified CHW Instructor preferred
Minimum Education:
High school diploma or equivalent
Minimum Experience:
Four years of community health worker or patient navigation experience required; supervisory experience preferred. May substitute required experience with equivalent years of education beyond the minimum education requirement.
Physical Requirements:
Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible amount constantly to move objects.
