Summary
We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team at Candlewood Healthcare & Rehabilitation. In this role, you will provide high-quality nursing care to our residents, ensuring their health and well-being while contributing to our mission of family-focused care.

Starting pay for this position, based on experience, is $28.50 an hour for LPNs and $40 an hour for RNs
The typical schedule for this opportunity is Weekend Warrior 8-hour shifts a week (Friday and Saturday). The standard schedule for this position includes an every other weekend rotation. Specific schedules available will be discussed during interview. The typical schedule for this opportunity is at Minimum two (2) 8-hour shifts a week. This position is eligible for our Weekend Warrior program working Friday/Saturday every weekend. By working the Weekend warrior program you will work two(2) 8-hour shifts but get paid for 20 hours. The shift times for this position are 7am to 3pm.

Responsibilities
Administer medications and treatments as prescribed by physicians.
Monitor patient health and provide updates to the healthcare team.
Assist in the development and implementation of individualized care plans.
Perform assessments, including vital signs and medical history evaluations.
Educate patients and families on health management and wellness practices.
Coordinate with interdisciplinary teams to ensure comprehensive care delivery.
Document patient progress accurately in electronic health records.
