Assertive Community Treatment (ACT) uses a multidisciplinary team approach to provide intensive mental health treatment services to individuals with severe and persistent mental illness in their homes and other community settings-24 hours a day, seven days a week, 365 days a year. ACT is often referred to as a hospital without walls and its goal is to keep individuals out of state hospitals, jails, emergency rooms and other institutions. ACT clients have to meet a strict criterion (i.e., diagnosed with a severe and persistent mental illness, unable to maintain activities of daily living, and be unsuccessful in other forms of treatment). ACT clients often have no resources and no family support and require an intensive and expeditious form of treatment. Typically, the goal is prevention of hospitalization or other out-of-home placements. Typically, the goal is prevention of hospitalization or other out-of-home placements.
Under broad supervision, identifies barriers to employment, refers clients to other support agencies, and assist clients/consumers with job readiness skills, job search strategies, and job interview preparation. Provides job placement and consumer/client employment assessments.

Duties & Responsibilities:
Develops and evaluates job skills and coaches for adults with behavioral health care needs who may be appropriate for community employment. Provides intensive intervention, treatment, rehabilitation, and support services to individuals with severe, persistent, and cute mental illnesses or addictive diseases. Provides as part of a comprehensive individual service plan, are designed to serve the acutely distressed individual in community with no delay, prevention hospitalization, incarceration, or other placements. Provides crisis intervention services to existing consumers as needed. Provides on-call, after-hours services on an as needed basis.Identifies possible job opportunities in the community and promotes the supported employment program through presentations and other community development strategies. Build Networks of employers interested in benefiting from the supported employment program and promote skills/abilities of individual candidates to employers.Performs a pre-placement assessment of the candidates job skills and interests. Develops an employment plan for the candidate that takes into consideration his/her needs, preferences, strengths and weaknesses in relation to job readiness, job acquisition, and job retention. Works with the candidate on job readiness skills: attendance, punctuality, hygiene, presentation, attitude, and communication. Provides job seeking skills training, such as use of the internet and publications to identify job opportunities, resume development, practice interviewing, and role playing/modeling. Connects the candidate to any resources needed to help him/her secure and keep employment, such as appropriate clothing, transportation, Vocational, Rehabilitation Services, or preliminary community classes.Participates as a member of the treatment team for individuals under their care. Participates in client-centered comprehensive assessment of psychiatric history, mental status, diagnosis, and physical health, education and employment and social development.Participates in the development of treatment and discharge plans based on observation of individual strengths, needs, abilities, and preferences. Provides effective follow-up and referral services that facilitate individual linkages, communication, and continuity of services. Facilitates individual-family interactions, keeping supervisor and other treatment team members informed of significant developments. Participates in daily staff organizational meetings and treatment planning review meetings. Interacts with individuals positively and respectfully. Uses age and gender appropriate language. Encourages individuals in appropriate social interactions that promote positive images and culturally typical behaviors. Facilitates individual skill-building in basic problem-solving and decision-making using the least intrusive methods appropriate for each. Acknowledges and praises their choices.Provides individual and group supportive counseling to individuals and families concerning their needs and problems, and issues with personal and social development. Leads educational and skill-building group sessions. Recognizes the need for crisis intervention and notifies professional staff of escalated situations.Plans and implements therapeutic, leisure and training activities that facilitate socialization and community integration. Schedules and transports individuals to appointments and community activities, such as shopping, banking, support groups, etc. Schedules follow-up appointments and makes referrals for community services.Monitors individual progress and compliance with treatment plan goals by one-on-one interviews and other contacts. Writes concise, accurate and clear observation notes for the individual record that document symptoms, behaviors, and skill deficits, the treatment interventions being used, and the individuals progress. Meets any applicable agency or payer requirements for regular individual contacts.Perform job site analysis and consultation with employers to identify and remedy barriers to employment. Provide on-site job coaching or other support necessary for success. Provide follow-up monitoring and consultation with the new employee and employer. Prepares required documentation and reports and maintains program files. Maintains regular contact with ACT team members regarding outcomes and issues that impact the individual services plan.Spends 45% or more of total scheduled work hours in the community, for example, meets with clients in community settings and develops relationships with managers at their businesses.Other duties as assigned.

Minimum Qualifications:
High school diploma/GED and three (3) years of job-related experience providing education, social or direct care services to persons with diagnosed mental/behavioral health, special needs, or other populations; or two (2) years of experience at the lower level Training/Employment Tech 1 (SST050) or position equivalent.

Note: Must be at least 21 years of age and have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy.

Requirements/Competencies:
Must be able to lift 20 pounds.Requires long periods of sitting/standing.Must have valid Georgia drivers license and Motor Vehicle Record in accordance with company policy.Candidates for selection must pass a criminal background check (including fingerprinting).Must pass a pre-employment drug screen and subject to random drug screens.
Note: Some positions may require a valid driver's license.
