Case Manager - Transitional Housing (Personal Services)
LifeWorks of Austin, TX is looking for a Case Manager to join our Transitional Housing program! The Transitional Housing Case Manager provides community-based case management services to youth and young adults (YYA) who are experiencing homelessness, housing instability, and/or early pregnancy or parenthood. This includes youth in the community through the 18-month Transitional Living Program (TLP) and expectant and parenting through the Young Parents Program (YPP). Services are delivered in both home and community settings, drawing upon clients' strengths to inform service planning, advocating on their behalf, and collaborating with other service providers to coordinate care, provide resource referrals, monitor progress, and manage crisis situations. Areas of focus include housing navigation and stabilization; life skills development; increasing parenting knowledge and skills; supporting infant and toddler development; strengthening co-parenting and family well-being; advancing educational and/or employment goals; connecting to community resources; pre- and postnatal care; breastfeeding; postpartum depression awareness; safe sleep practices; fostering healthy parent–child relationships; and promoting overall stability and independence.
This full time, non-exempt position earns a competitive starting rate of $24.86-$27.17/hr. depending upon experience. Benefits are also included, which consist of 24 PTO days per year (accrued); 11 paid holidays per year; a premium-free employee medical plan with $100 per month HSA contribution (plus other affordable employee or family medical, dental, and vision options!); a retirement savings plan with 50% match on the first 6% of contributions, and much more!
