Are you a detail-loving, team-leading, assessment guru? Roseville Care Center is looking for an experienced and passionate MDS Lead to take the reins of our clinical assessment process and help ensure every resident receives the high-quality, person-centered care they deserve!

🩺 What You'll Do:
As our MDS Lead, you'll coordinate and oversee the Minimum Data Set (MDS) process, ensuring accuracy, compliance, and collaboration across departments. You'll work closely with interdisciplinary teams to develop and maintain individualized care plans, while guiding the facility's reimbursement accuracy and quality measures.

💼 What We're Looking For:

• Licensed LVN or RN in the state of California
• At least 1-2 years of MDS experience (lead experience preferred!)
• Strong knowledge of PDPM, care planning, and clinical documentation
• A team player with leadership skills, attention to detail, and a heart for long-term care
