Pine Creek Care Center is seeking a dedicated and compassionate Licensed Physical Therapist to join our interdisciplinary care team. In this full-time role, you'll help patients regain mobility, restore function, and enhance their quality of life through personalized therapy and evidence-based interventions. Whether you're an experienced clinician or a new graduate, we offer a supportive and collaborative environment where you can thrive professionally.

Key Responsibilities:

  • Patient Evaluation & Care Planning:

  • Conduct comprehensive physical therapy assessments.

  • Develop and implement individualized treatment plans in coordination with physician orders.

  • Collaborate with referring physicians and healthcare team members to ensure continuity of care.

  • Direct Treatment & Supervision:

  • Provide skilled therapy sessions tailored to patient needs and goals.

  • Supervise Physical Therapist Assistants, therapy aides, and students per state practice standards.

  • Assist in the training and guidance of Restorative Aides in collaboration with the nursing department.

  • Team Collaboration:

  • Participate in care planning meetings, utilization reviews, and interdisciplinary team conferences.

  • Effectively communicate patient progress, challenges, and discharge planning with the care team.

  • Documentation & Compliance:

  • Complete evaluations, daily notes, weekly progress reports, and recertifications using NetHealth documentation software.

  • Ensure all documentation meets state, federal, and third-party payer regulations.

  • Family Training & Discharge Support:

  • Educate patients and caregivers on home exercise programs and safety practices.

  • Recommend and help coordinate the provision of durable medical equipment (e.g., walkers, braces, grab bars).

  • Equipment Oversight:

  • Report equipment issues to ensure a safe and effective therapy environment.

Qualifications:

  • Education:

  • Bachelor's degree in Physical Therapy required.

  • Master's or Doctorate in Physical Therapy preferred.

  • Licensure:

  • Current Physical Therapist license in the state of California.

  • Must remain in good standing and meet continuing education requirements.

  • Skills:

  • Strong clinical, organizational, and communication skills.

  • Experience with Net Health (or similar EMR platforms) preferred.

  • Team-oriented mindset with a passion for quality care.

Physical Demands:

  • Frequent walking, standing, reaching, and lifting.

  • Ability to occasionally lift or exert over 100 pounds.

  • Occasional stooping, kneeling, or crouching required .

  • Must have adequate vision, hearing, and mobility to perform duties.
