Admissions Coordinator (Project Management)

Job Summary:
Responsible for overseeing and managing census and admissions processes to meet facility goals. Builds and maintains strong relationships with residents, families, referral sources, and the community to support census development and facility reputation.

Key Responsibilities:

Manage admissions, discharges, benefit verification, and census objectives.

Develop and maintain referral networks and community partnerships.

Generate leads, track demographics, and report census activity.

Communicate effectively with staff, residents, and outreach contacts.

Support facility marketing, outreach programs, and events.

Maintain accurate documentation and reports.

Assist with quality assurance, surveys, and policy updates.

Ensure compliance with all regulations and facility standards.


Qualifications:

Strong communication and interpersonal skills.

Highly organized with attention to detail.

Ability to work proactively and collaboratively.

Enthusiastic, professional, and community-focused.


EDUCATION and/or EXPERIENCE:
• RN, LPN, or social work background. Meets state and federal requirements.• Associate or Bachelor's degree (preferred)
CERTIFICATES, LICENSES, REGISTRATIONS:Nursing license preferred.
